You have to determine where the sound is coming from, inside the engine or
out.

Remove the belt(s) when the engine is cold and start it.  It won't harm the
engine if it is run for only a short period.

If the noise goes away, it is from the alternator, water pump, or air pump.

If the noise is still there, it is internal, and the engine will need to be
torn apart.

BTW, what is the oil pressure when the noise occurs?
