The ground wire is usually secured under the screw that holds the points
plate base in place.  This is recessed into the body of the distributor (the
ones I have at any rate) and needs no cut-out in the cap to clear them.
There *is* a recess in the cap, but that is for the locating peg, so it can
only be fitted in one orientation.
