One solution I have used in the past is:
-remove the nut,
-sit in the driver's seat,
-pull on the steering wheel as hard as possible,
I guarantee it will come off.
Your dentist will be able to assist you with the remaining repairs.

Another method I have used is to use a hub puller, or the like, to 
loosen the steering wheel nut but leave it on so that you can skip the 
orthodontist visit.
