I'd say it is moderately difficult because the wire cable that connects the 
t-handle to the detent on the bottom of the shifter is brazed to the bottom.  
Removal requires unbrazing the wire end, pulling it out the top and then 
reassembly - either brazing or as someone recommended drilling/threading a 
hole in the bottom of the shifter and usign an allen screw to fasten the 
cable.  If you have a torch, not too bad.  If you don't very difficult.
