I always used a channel lock pliers.  Grab on with the pliers pupendicular
to the wheel, hold it tight with my left hand half way up and hit the end
with the palm of my right hand.  If that dosesn't work right away, grab on
parralel to the wheel and try to rotate and wiggle it loose.  As a last
resort, I call it all kinds of names until its so embarrassed, it falls off.
