I think the brake issue is separate, probably caused either a vacuum leak or
by an intermittent failure in either the booster or the master cylinder.
As for all of the codes, I suspect operator error unless the car died before
the goose test was signalled. Remember, you only have 10 seconds to hit the
brake, goose the throttle, turn the steering wheel, and cycle the overdrive
switch when the CEL flashes. It takes some practice.
