I would guess either that adjuster or the cable. Per your description, it
sounds like it grabs OK. Another test is to start out from a stop in 2nd,
then 3rd gear. Get the clutch out quick, and accelerate. If it's slipping,
you will know it.

That was my first thought and I released the ratchet mechanism only for it
to adjust its self back to the same location.

It's an adjustment problem. The clutch should not let out at the very top.
If the lift trick isn't working then maybe there is some problem with that
part of the mechanism.
