Unfortunately, it's never an adjustment problem (OK, one in a million).
Something is worn out, either the clutch, pressure plate, or both

It's an adjustment problem.  The clutch should not let out at the very
top.  
If the lift trick isn't working then maybe there is some problem with
that 
part of the mechanism.
