[Shotimes] Surging/Power Loss

I have a 91' with 99K on the engine and I have a little problem.  After the 
engine has had an opportunity to cool off completely upon restarting and 
then accelerating for the first time the car surges and has a power loss up 
to 4K on the tach.  It is most noticable in 2nd gear.  The thing that 
baffels me about it is that after I have run it up the speed limit say 45 
mph it and then come to a stop the next acceleration is almost always 
normal.  I honestly am not sure where to start on this one.  I have replaced 
the MAF and that didn't help.  Any suggestions?
