Likely to be just the shifter, or rather the ball-and-socket on one 
of the shifter cable has fallen off. I had this problem on my Taurus 
MT5 which is basically the same as the SHO cable shifter. You can get 
the shifter cable back on and hold it in place with a couple of 
tie-wraps. Long term, your best solution is to replace with a 
rod-shifter.
