 I have had this several times, just about the exact same symptoms. Reseat
the LPM. I have found that over time the LPM fit into the PCM does get
worse. The LPM is very light weight but is heavy enough to work on the plug
I guess? I duct tape the ribbon cable to the PCM as tightly as possible. You
can't get at it very good to secure it anyway!
