[Shotimes] What Cause a car to run too lean?

Other causes for lean condition:
- dirty fuel filter
- dirty injectors
I was also gonna say a failing fuel pressure regulator, but wouldn't that normally cause a rich condition? Or can it fail in such a way as to create a lean condition?
