If the vacuum diaphram were to develop a leak, it would cause the
regulator to be in low pressure mode all the time.

I was also gonna say a failing fuel pressure regulator, but wouldn't
that normally cause a rich condition? Or can it fail in such a way as
to create a lean condition?
