Sounds like the TOB to me. You still apparently have the old design that plugs up
with dust and dirt from inside the bell housing. The new ceramic TOB stays clean,
and won't eat the fingers of the pressure plate, which is what the siezed up old
bearings do.
You need a clutch, and get the new TOB. More expensive by a bit, but will last much
longer. Mine has 80K miles and still feels new.
