Jim,
for this application, I would think you could get very good results if you
could get ALL the rust off (sandblasting?) and then use JB Weld to
smoothen the surface out.  JB Weld might be similar in many respects to
the "like metal" products discussed earlier.

Yes, I have fixed a 1" by 2" hole in an engine block with JBW and it is
still going strong.
