I did mine with the engine in the car.  It took longer than 5 minutes, but it 
wasn't a horrible project.  I microwaved the bushings a little to make them a 
little pliable.  A not so BA screwdriver did the trick working little by 
little.  Flexing the throttle shaft to get it back in the hole was the hardest 
part.

I just got through with this job, and find myselft
wondering what's the big deal? First, to answer your
question, on my car the flat (thick) side was in the
engine compartment and the tapered side was in the
wheel well, so I installed the new one that way. 

I went to considerable trouble to prepare (put in hot
water, tried the socket trick recently described)
since I had read many times how difficult it was. The
socket trick did not work - it compressed the bushing
without forcing it through. So I decided to make a
straightforward attack on the problem. I worked from
the engine bay side, so I was pushing the thinner side
through, got a portion started, and worked the rest
through with a screwdriver. Done, in less than 5
minutes. This makes me wonder why everyone who
describes this in detail seems to work from the
footwell and push the thick side through. I was
working with the engine out of the car - is that what
made the difference? (I tried looking at my photos
with the engine in, but couldn't find anything that
clearly showed how much working room there was.)
