Everyone is focusing on the input shaft bushing but I happen to think it's
something else.

Did you make sure your release bearing was pressed on in the correct
direction?

Was the release bearing collar assembled on the tranny correctly?

Was the clutch disc put on correctly?

Are you sure the clutch package is correct for the car?

Finally - is there any chance some 4 speed clutch parts could have been
interchanged?

PS - Revving an engine to 7000 RPMs with no load on it doesn't do your
engine very good, especially your crank.
