Greg,
I don't know if there is an industry standard, but in general, I imagine 
typical rebuilders try to bring a rebuildable unit back up to a minimum of 
65-75%. 
Otherwise they'd have to use too many brand new parts.
Since there are NO new STOCK racks and NO new STOCK pinions available for 
TR4-6 range (that I know of) 'brand new' is not even an option here. Only 'used 
parts' from stripped cores are typically used.
You can trust a rebuilder based on their reputation or you can rebuild your's 
yourself or you can buy a brand new aftermarket substitute.
Asking a rebuilder what is replaced/renewed is a tough question - especially 
if typically, like on these steering racks, nothing might be replaced or 
renewed but simply checked, readjusted, painted, and lubed. And if something is 
replaced, it is likely replaced with an already used part. Any vendor will have 
a 
hard time describing 'how used' the particular parts in any one rebuilt unit 
are.
If you want the highest quality I believe it will cost you 3 times the cur
rent market rate. (time finding NOS or NLA parts, paying for these parts, and 
time doing the rebuild yourself)
Or you (or a rebuilder of your choice) could find nothing inside your rack 
needs replacement but simply an adjustment of the shims to regain compliance 
with original factory specs.
