Sounds like the lever has streached. A new lever may fix the problem. For a quicker turn-around you can enlarge the gap at the open end of the lever with a cut-off wheel on your Dremel(R) roto tool.

Sounds like it to me. Not sure I understand this question ... isn't the washer/distance piece/collar/whatever a "piece which slips over the shaft" ? Unless you've disassembled it, the O-ring is not
