I usually use two combination wrenches.  You can get the box end
of one wrench on one side, and maybe the box end of the other one on
as well, if it is thin enough.  But you do have to wiggle a bit to
get a good grip.  I try to get the wrenches aligned such that I can
squeeze the handles together using both hands to break the bolts free.
Of course, I don't have the grip strength I had 25 years ago when I
was climbing all the time and making a living (ha!) sewing up backbacks
and big wall haul bags out of heavy ballistics nylon.

Well, yes.  Ideally one would get original style bolts that have a solid
grip length where the fastener passes through the flanges, but in truth
there are no doubt quite a number of TRs out there with basic hardware
store bolts that have threads along the portion of the bolt in the flanges.
And you will want to use new locking nuts here - the usual problem with
these is having them come loose, not be too tight!
