Hi John,

This sounds to me to be a classic case of worn butterly shafts that 
sometimes allow the butterfly to come completely closed and sometmes move a 
bit and cause the butterflies to remain open. You can check the shafts 
carefully by overcoming the return springs' attempt to hide the slop and 
trying to wiggle the shafts.
