I think you forgot the beer sacrifice...:->

Sounds like either just plain bad luck or a defective head gasket. Perhaps
a piece of debris got in there  without you noticing. Having done this
probably a dozen times over the years on different cars, I can't recall
this ever happening to me. Seems like you did everything right, but you'll
be able to tell more when you break it down again. I would check to make
sure that you don't have a cracked cylinder head too. That would have
simailar symptoms.
