A couple of these questions are typically for the British market.
In the UK a registration number and registration document is issued and
stays with the car the entire life of that vehicle. Therefore you could find
the date of registration and the registration number from this document if
you were in UK. The only thing that can ever changes on the reg. doc is the
registered keeper, colour of vehicle and the engine number. (there is
provision for private registration numbers but this is in the minority).
The 'Issued to' question is your name, each certificate is personalised.
