Your Idle Air Bypass Valve is the first culprit I'd look at.  It's round, about 4-5
inches long or so, and mounts on the back of the throttle body, with two hex screws,
and has a plug on the firewall side.  It is operated by the PCM and allows air to
flow past the fully closed throttle in a very accurately measured amount to keep
pollutants low at idle.  They stick open, or fail electronically when they get old. 
It sounds like yours has failed electronically, if it is in fact the problem.  90%
of the time, bad idle is because of the IAB.
