The first thing that comes to mind is the IAB.  How old is it?  Have you got one you
can swap onto the TB to see if it will work better?

The IAB's main job is to give air for a stronger idle when A/C or heavy alternater
type loads hit the engine when it is idling, and they do wear over time.
