John, what I have always used, believe it or not, is a slide hammer (like one
you use to pull dents) with a suitable bolt securely taped to the end of the
hammer.  Thread the bolt into the main cap in the hole provided, give it a few
whacks (or slide hammer pulls upward), and out they come.  Hope this helps!
