1.  Who knows?  They do break from time to time, although rarely in my
experience.  A dry and sticky cable will probably make it more likely.  The
squared ends are hardened to resist abrasion, but this makes them more
liable to fracture.  If the outer was making a tight radius to the
right-angle drive or not fully screwed to it it could have caused the
squared off inner to be bending back and fore slightly as it was turning,
eventually fracturing it.

2.  If the cable still turns in the sheath it wasn't that, if the input
shaft still turns it wasn't the speedo head either.  One or the other
*could* have jammed enough to break the inner then freed itself, but it
seems unlikely.  Changing the cable will eliminate that as a possible
reoccurrence.

3.  No.  If the plastic gear jammed it would have stripped, the inner and
speedo would just have stopped turning.

4.  Just make sure the input shaft in the speedo head turns very freely,
there should be virtually no resistance.  If not already then lubricate the
new cable.  You will get nearly as many opinions on what to do it with as
there are listers still awake.
