Most likely dirt or grit on the bendix drive.
Next time it happens, just rock the car in 4th gear, it will pop out.
I have this problem more then I like due to the 5 speed and the bendix hanging
out in the open. It picks up all the dirt.
A good cleaning and oiling of the starter drive usually cues the problem.
A bad bendix spring will cause it too.
