David,
I would remove the differential from the car and with the correct size
Allen wrench, remove the bolts.  They should easily be removed when the
axles are off the car.  Then make sure you have good lock washers on
each one and install them with a little lock-tight.  You shouldn't have
any more problems with them coming out by themselves.
