Not uncommon at all [To find loose or missing bolts]!

One thing, is back in the late 60's & early 70's many mechanics
wouldn't pay attention to METRIC or SAE bolts, and put what ever
seemed correct into the various locations. With the bolts holding
the starter, that was really a pain as they threaded right into
the aluminum tranny housing!

I like the "positive lock" stainless steel inserts. You drill out
the hole, thread it, install the insert, then "stake" the locking
pins down. Viola a bolt hole that will not thread out. I prefer
those over helicoils (which seem to back out at the wrong time.
