 Maybe this has been addressed already but I have seen rocker arm ends
become concave after much contact with the valve stems.  A feeler gauge
won't detect this so you won't get a true gap setting the ordinary way.
The fix for this is to grind the rocker faces smooth.

On a trip from Texas to Daytona Beach a number of years ago the nut came
lose and I swore it was valve train noise.
