I was able to get to this bolt with a long extension on a socket from under
the car with the car up on jackstands.  The rachet was then below the floorpan
and free
to turn 360 degrees.  And yes it is possible to remove the manifold with the
head in place, but it is really tedious, only a fraction of a turn at a time
on the lower
bolts.
