The current issue of Consumer Reports deals with both patient and doctor 
complaints about each other.  One of the doctors' biggest complaints?  
Patients who *insist* on having certain specific tests performed, even 
though the doctor does not see it as either necessary or helpful.  The 
result?  Higher costs.

     --Other patient complaints:  "kept me waiting for 30 minutes or 
longer", "could not schedule an appointment within a week", "spent too 
little time with me", "didn't get me tests results promptly", "didn't 
respond to my phone calls promptly".

     --Other doctor complaints:  "don't follow their prescribed 
treatment", "wait too long before making an appointment", "request 
unnecessary prescriptions".

Notice how many of these relate to costs.
