Ken,
Could be a,b,or d, remember there are no stupid problems when it comes
to wiring and electrical problems.  Look for the problem as soon as you
can before it creates an even bigger and costlier problem.  Your chances
of "a" are extremely unlikely, but stranger things have occured.
