Compression testing question

 I have a question (IHAQ) .
 What is the best way to do a compression test?
 
 Should I remove all the plugs and rotate the engine by hand?
 Use the starter?
 Only remove one plug and start the engine?
 What RPM is used?

 Since any one of the above will work (and all will produce different results)
 What is everyone else doing?

 OH, and I realize its more of an issue of consistenticy between cylenders
 rather than actual PSI.
