Dave-

What worked for me was to just mount the arm in a
vise,  and grab the largest drill bit which fit
between the inner sleeve and the inside of the hole in
the arm.  

I just drilled a series of holes (6-8?) through the
bush around the inner sleeve, then drove the whole
mess out with a hammer & dowel.

Each bush took about one minute using this method :-)
