Sorry if this is a repeat of what someone else said, but to locate the stock
holes, look on the inside of the cowl for the original mirror holes on the
stiffener plate that backs the cowl sheet metal (sometimes a PO will cover
over the drivers side holes on the cowl sheet metal and the passenger side
is typically not drilled yet).  Drill the top hole as a "guide hole" from
the inside out (using a very small diameter drill bit) and then use the
mirror as a template to drill the bottom hole for proper spacing (from the
outside - you'll find there isn't enough room to drill the bottom hole from
the inside to out).  Then use your final size drill bit to finish the holes.
This is really important on the passenger side or the hood will mack the
mirror.  And it fits the holes properly on the stiffener plate for strength.
Use a thick rubber gasket (thickness of tire tube) under the mirror to seal
the holes and even out the mounting surfaces of the mirror and cowl sheet
metal (reproduction mirrors are typically of mediocre quality). Good luck!

Paul, I mounted my mirror without the aid of an "assistant".  Just held the
mirror to the cowl, marked the first hole with a pencil, then drilled the
hole.  I did drill a "practice hole" in a piece of sheet metal first just to
be sure it was the right size for the mounting screws.  After the first
hole, the second one is obviously easy.  One word of caution....I mounted my
mirror a bit too far forward on the cowl.  The hood bumps the base of the
mirrow when it's raised.  I needed to mount the mirrow about an inch futher
back than I did.  (Too late now, but no major problem.)  Might help to raise
the hood when mounting the mirror just to be sure it doesn't interfere with
the mirror when you put it on.  BTW, I got my mirror and mounting kit for
$15 from the Truck Shop.  Found it on special at an auto show.  Haven't seen
that good a price since.  Good luck!
