Was the bottom end looked at?  I had piston slap occurring in my engine that
I had originally thought was a valve train noise.  It would smoke a bit and
idle rough too. All a result of the rings not sealing very well.  You may
want to put a vacuum gauge on it.  My car only made about 13" of vacuum with
an A cam.
