As far as centering the drill, I have a set of Snap On screw extractors with a
set of guides included. Basically they are a piece of round stock turned to the
OD of the hole with a small pilot hole through the middle to drill the center of
the stud. Very easy to make in a lathe and harden it with a torch. Only good if
the broken stud is recessed enough to enter the guide in the hole.
 The opposite can be made for the exposed stud.  Drill a pilot hole through a
piece of metal and recess one end the depth and diameter of what sticks out. The
recess holds over the stud and centers it while drilling the pilot.  At least in
theory. I have had good luck centering  with this method.
