I have had a jack stand fail.  It was one of these cheap ones with
the teeth that a little lever slides into to lock it in position.  The
little lever broke sending the jack stand plummeting down to its lowest 
setting.  Luckily, I wasn't under the car and the car didn't fall off of the
jackstand.

I now have the kind with the large clevis pin that goes through both
pieces.
