The nut did not fail, but it did come off. I think the cotter pin must have failed to allow this. I think the shop may have tightened the bearing too much, but there is no way to prove this. The whee

In my catastophic case, the spnidle broke at the bearing that froze. The nut and bearing (presumbably the washer, too) and the end of the spindle were welded together. I tried to separate them later

I don't know. We didn't find one, but it certainly could have escaped. The shop adjusted the bearing, but I don't know if they took off the nut to check on the D-washer. Dunno. Didn't recover it. I
